STUDIO SCREENING

Regular physical activity can be fun and beneficial for your health. Increased activity for most can be safe. However, some people should consult their Doctor before increasing the amount of exercise they take.

If you are planning to become considerably more active than you are now, please start by reading the following statements and answering the questions below –

~ If you are over the age of 69, don’t exercise regularly and unfit – Please consult your Doctor

~ If you are between the ages of 15 and 69 and answer YES to any of the questions below – Please consult your Doctor

Please consider the questions below and let your Instructor know of any concerns before taking part in your Exercise Class.

1. Has your Doctor ever said that you have a heart condition and that you should limit your physical activity?

2. Do you feel pain in your chest when you undertake physical activity?

3. In the past month, have you had chest pains when NOT undertaking physical activity?

4. Do you lose your balance because of dizziness or do you ever lose consciousness?

5. Do you have a bone or joint problem (back, knee, hip, neck) that could be made worse by a change in your physical activity?

6. Is your Doctor currently prescribing drugs for high blood pressure or a heart condition?

7. Do you know of ANY OTHER REASON why you should not undertake physical activity?

I am in full understanding of all the relevant questions, and will advise my Instructor of any concerns before participating in any Studio Classes. I also understand that I will not use any item of exercise equipment without receiving full instructions before hand.
